Health In Balance Physical Therapy is proud to say that we offer the best physical therapy services in the area, and you will receive individualized treatment sessions with someone who really listens to you. The information below will help you to prepare for your first appointment with us.

To Schedule An Appointment:

In Wisconsin, there is direct access to physical therapy, which means you can come directly to a PT without having to see your physician first.  At Health In Balance Physical Therapy we work with you, your primary care physician, and your orthopaedist.  If you do not have a doctor, and it is indicated to see one, we can assist in getting you to see the appropriate physician, usually within the week.  Your insurance may require a prescription from your doctor before they will pay any benefits.

If you do not have a prescription and are unsure if physical therapy is appropriate for you, we offer a 15 minute free injury screen.  Through this screen we assess your condition, determine your needs and begin a dialogue with your physician to attain a prescription to begin physical therapy swiftly.

If you have already scheduled an appointment with us you may print out the forms below so that you can save yourself time when you come for your first appointment.  If you have not made an appointment, please call us at (262) 236-0176 or use our online Request an Appointment form.

Insurance:

Please call our office so that we can verify your insurance. If you don't have insurance coverage or if you have an extremely high deductible and wish to not use your insurance, you may be seen as a cash patient and pay before each appointment.  Our Prompt Pay rates are $150.00 for the initial evaluation visit and $140 for each hour follow up visit or $70.00 for a half  hour follow up visit .  We accept cash, personal checks, VISA and MasterCard.

What to expect on your first appointment:

In a few days you will have your first physical therapy appointment.  You should arrive 10 minutes early for your appointment and wear clothing that is loose fitting. Please fill out the registration forms, provided above, and bring them with you to your evaluation. Your therapist will be asking you many questions regarding your problem. She will also take range of motion measurements, test your strength, and perform special tests as it relates to your diagnosis. The therapist will gather as much information from you through the evaluation process to formulate an individualized therapy plan. Your point of view and goals for recovery are important.  You will also be given exercises to work on at home which are imperative to your recovery.

It is normal to experience some soreness, increased pain, or a change in your pain location when first starting therapy. As you get stronger this will diminish and you will be able to tolerate more advanced exercises. You will also notice that life tasks will be less painful and easier for you. Your physical therapist will continually reassess your progress toward your treatment goals and will work with you and other health professionals to plan for your discharge from physical therapy.

Thank you for choosing Health In Balance Physical Therapy to take you through your therapy journey.  We promise to be there for you every step of the way, and help you towards a full recovery.
